Sanasign Signer Terms

You have received a secure link to review and sign a clinical document sent by a home health agency, hospice, or other organization (the "Sending Organization") through Sanasign, a clinical document workflow and electronic signature service operated by Sanasign, Inc. ("Sanasign," "we"). By signing, declining, or otherwise acting on the document, you agree to these Signer Terms.

1. You are the signer. This link is personal to the provider identified in it. By proceeding, you confirm that you are that provider. Do not forward or share the link, and do not permit anyone else — including your staff — to sign on your behalf. If you received this link in error, or the patient identified is not under your care, do not sign: use the decline option or contact the Sending Organization, and you may notify us at [email protected].

2. Your signature is yours. Signing through Sanasign is the same act as signing on paper. By signing, you attest that you have reviewed the document, that its content is accurate to the best of your knowledge, and that you are the provider ordering or certifying the care described. The document's clinical content was prepared by the Sending Organization, not by Sanasign; review it before you sign.

3. Electronic signature consent. You consent to review and sign this document electronically. Your electronic signature has the same legal effect as a handwritten signature under the federal ESIGN Act and applicable state law. Sanasign records your identity, the date and time of your actions, and an audit trail, and provides them to the Sending Organization. If you prefer to sign on paper, do not proceed; contact the Sending Organization.

4. Confidentiality. The document contains protected health information. Use and disclose it only for treatment, payment, and health care operations, and handle it in accordance with HIPAA and your own organization's policies.

5. The service. The link is secure and expires. Sanasign provides the signing workflow as-is and is not responsible for the document's clinical content; your relationship regarding the underlying care and documentation is with the Sending Organization and the patient. Your use of any other part of the Sanasign website or service is governed by our Terms of Use (www.sanasign.com/terms) and Privacy Policy (www.sanasign.com/privacy).

6. No charge. There is no fee to review or sign. If your practice would like its own Sanasign account — including a dashboard of pending requests — visit www.sanasign.com.
