Sanasign Authorized User Terms

These Authorized User Terms ("these Terms") apply to you as an individual member of an organization's workforce activating a Sanasign account. Your organization's use of Sanasign is governed by the Sanasign Healthcare Provider User Agreement (the "User Agreement") between Sanasign, Inc. and your organization; these Terms govern your individual access. By activating your account or using Sanasign, you agree to these Terms.
1. Your identity. The account is yours alone. You confirm that the name, credential, and professional information associated with your account identify you truthfully, and you will keep them current.
2. Your credentials. Do not share your password, one-time codes, or any other credentials, and do not use anyone else's. We strongly recommend enabling multi-factor authentication. Tell your administrator and [email protected] immediately if you believe your credentials have been compromised.
3. Signatures are personal. If you sign a document in Sanasign, you are attesting that you are the person identified as the signer, that you have reviewed the document, and that you are personally taking that action. Never sign on behalf of another person, and never allow anyone — including your own staff — to sign on your behalf. Preparing and reviewing documents may be delegated; signing may not.
4. Access what you need. Access, use, and disclose information in Sanasign only as needed for your role and as permitted by your organization's policies and applicable law, including HIPAA. Patient information you see in Sanasign may be used only for treatment, payment, and health care operations.
5. Confidentiality. Keep information you access through Sanasign confidential in accordance with your organization's policies and the law. Do not export, copy, or share it except as your role and the law permit.
6. Acceptable use. The use restrictions in the User Agreement and the Sanasign Terms of Use (www.sanasign.com/terms) apply to your individual use. Do not attempt to access anything you are not authorized to access.
7. Administration. Your organization controls your account and may modify or revoke your access at any time. We may suspend or terminate your access for violation of these Terms, and may monitor use of the service as described in the User Agreement. When you leave your organization, your access ends.
8. Electronic transactions. You consent to transact electronically, including applying electronic signatures, which have the same legal effect as handwritten signatures.
9. Relationship. These Terms do not make you personally responsible for your organization's fees, and do not create any agreement between you and Sanasign other than as stated here. The User Agreement — including its dispute resolution, disclaimer, and limitation of liability provisions — governs the service itself.
